ABSTRACT

ObjectiveTo investigate the effect of hyperbaric oxygen combined with dynamic scalp acupuncture on motor function for stroke patients with hemiplegia, and compare the effect of dynamic scalp acupuncture inside or outside hyperbaric oxygen chamber. MethodsFrom January, 2021 to June, 2022, 72 stroke inpatients with hemiplegia in Fuyang People's Hospital were randomly divided into control group (n = 24), combination group 1 (n = 24) and combination group 2 (n = 24). All the patients received routine treatment, while the control group received dynamic scalp acupuncture, the combination group 1 received hyperbaric oxygen and dynamic scalp acupuncture outside the hyperbaric oxygen chamber, and the combination group 2 received hyperbaric oxygen and dynamic scalp acupuncture inside the hyperbaric oxygen chamber, for four weeks. They were assessed with National Institutes of Health Stroke Scale (NIHSS), modified Barthel Index (MBI), Berg Balance Scale (BBS), and Fugl-Meyer Assessment (FMA) before and after treatment. The rest pulse oximetry saturation (SpO2),the lowest SpO2 (SpO2L) and the variation of SpO2 (ΔSpO2) were measured in the first and last treatment. ResultsThe scores of NHISS, MBI, BBS and FMA increased in all the groups after treatment (|t| > 7.681, P < 0.001), and they were more in both the combination group 1 and the combination group 2 than in the control group (P < 0.05). During the first and last treatment, the rest SpO2 and SpO2L increased in the combination group 2 compared with the control group and the combination group 1 (P < 0.05), while ΔSpO2 decreased (P < 0.05); and SpO2L increased in the combination group 1 compared with the control group during the last treatment. ConclusionThe combination of hyperbaric oxygen and dynamic scalp acupuncture can significantly improve motor function and oxygen-carrying capacity in stroke patients with hemiplegia.

ABSTRACT

Objective:To analyze retrospectively the risk factors for pulmonary infection after traumatic cervical spinal cord injury.Methods:The 154 patients with a cervical spinal cord injury studied included 120 with a pulmonary infection and 34 uninfected controls. Regressions were evaluated using data on their genders, ages, the cause of injury, affected segments, the neurological level of the injury (NLI), and the presence of a vertebral fracture or dislocation.Results:Age, complete injury, NLI at C 1 to C 4, and an injury-to-treatment time of more than 8 hours were found to be independent risk factors for secondary pulmonary infection. Conclusion:Elderly spinal cord injury patients, with a complete injury, an NLI between C 1 and C 4 or an injury-to-treatment time of more than 8 hours have a higher risk of pulmonary infection.

ABSTRACT

Objective:To evaluate the effect of "simulated rehabilitation team" teaching model on the learning effect of clinical interns through retrospective analysis.Methods:A total of 124 rehabilitation undergraduate students were divided into control group ( n=58) and experimental group ( n=66) according to the order of practice. The control group received the traditional rehabilitation teaching mode. The experimental group adopted the "simulated rehabilitation team" teaching model, and the students were given theoretical and practical examinations 3 months before and after the end of the practice. Results:There was no significant difference in the theoretical test score between the experimental group and the control group ( P>0.05) three months before and after the practice, but there was significant difference in the practical test score between the experimental group and the control group ( P<0.05). After the practice, the scores of students in the experimental group were better than those in the control group ( P<0.05). Conclusion:Compared with the traditional rehabilitation teaching model, the "simulated rehabilitation team" teaching model can significantly improve the students' theoretical and practical achievements, and improve the students' mastery of theoretical knowledge and comprehensive practical ability.

ABSTRACT

OBJECTIVE: To investigate the role of clinical pharmacists in the individual anticoagulation of warfarin for the patients with lower extremity venous thrombosis (LEVT) and pulmonary arterial thromboembolism (PATE). METHODS: Clinical pharmacists participated in individual anticoagulation of warfarin for the patients with LEVT and PATE. It was suggested to detect the gene type of the patient. According to the results of gene test [cytochrome P450 (CYP)2C9*1*1 and vitamin K epoxide reductase complex subunit Ⅰ] and the dose recommended by FDA based on the patient’s gene, the initial dose of warfarin (3.125 mg,once a day) was determined according to the patient’s living habits, height and body mass. Then the maintenance dose of warfarin (the maintenance dose of warfarin was 2.5 mg and 3.125 mg, once a day, alternately taken every other day) was calculated according to the warfarin maintenance dose prediction formula established by Warfarin Pharmaeogenetics Consortium. Pharmaceutical monitoring was conducted, such as INR, prothrombin time and bleeding event monitering. RESULTS: Physicians adopted the suggestion of clinical pharmacists. The maintenance dose of warfarin was 2.5 mg and 3.125 mg, once a day, alternately taken every other day. It was suggested to give Flucloxacillin sodium injection which had less influence on warfarin. The patient recovered well and was discharged. CONCLUSIONS: Based on pharmacogenomics, clinical pharmacists participate in the formulation of individualized anticoagulant regimens for patients, which promote TNR ralue of patients, reduce the risk of early postoperative thromboembolism, and further ensure the safety of drug use in patients.

ABSTRACT

Objective To observe the clinical effect of surface electrical stimulation on bladder spasm among patients with spinal cord injury,and compare it with that of therapy combining electrical stimulation with bladder function training.Methods Forty-two patients with bladder spasm after spinal cord injury were randomly divided into an experimental group (n=21) and a control group (n=21).Patients in the experimental group were further divided into three subgroups:patients with cervical spinal injury (n =7),thoracic spinal injury (n =9) and lumbar spinal injury (n =5).Both the experimental group and control group were given normal bladder function recovery exercise,while the experimental group was given the extra electrical stimulation on the surface of the sacral nerve.Urodynamic tests including the maximum detrusor pressure,bladder capacity,residual urine volume and bladder compliance were conducted for both groups before the treatment,after the first and the second 18-days of treatment and during a follow-up visit 2 months after the intervention.Results After the first course of treatment,significant differences were found in all measurements in the experimental group and most measurements of the control group except for the residual urine volume.Significant differences were found in all measurements after the second course of treatment compared to those after the first course in both groups.After the two courses of treatment and during the follow-up visit the average residual urine volume of the experiment group was significantly better than that of the control group.After the second course of treatment the average maximum detrusor pressure and bladder compliance of the experimental group were significantly better than those of the control group.Conclusion Surface electrical stimulation significantly improves the urodynamics and bladder function of patients with bladder spasm after spinal cord injury and its therapeutic effect is greater for patients with cervical and thoracic spinal injury than for those with lumber spinal injury.

ABSTRACT

Objective To investigate the effect of MOTOmed movement therapy on balance and ability in the activities of daily living (ADL) of convalescing stroke patients with hemiplegia. Methods Forty convalescent stroke patients exhibiting hemiplegia were randomly divided into a treatment group and a control group with 20 cases in each group. The control group received routine rehabilitation training; the treatment group received MOTOmed training in addition. Balance function of all the patients was assessed using Berg's balance scale (BBS) , and the Barthel Index ( BI) was used to assess ADL ability at the beginning of the program and after 6 weeks of treatment. Results Balance and ADL ability improved significantly in both groups after 6 weeks of treatment. The effect in the treatment group was significantly better than in the control group. Balance and ADL ability were positively correlated. Conclusions Applying MOTOmed therapy along with routine rehabilitation training can distinctly improve balance and the ADL ability of hemiplegics after stroke.
